Too many New Yorkers on Medicaid don't know free help is right there for them. We exist to close that gap — one phone call, one form, one neighbor at a time.
SCN Direct is an independent community outreach organization. We are not a government agency and not one of New York State's official Social Care Networks (SCNs). What we do is simpler: we listen to people in our community, ask what they need, and connect them with a Navigator inside a contracted SCN who can deliver real help — at no cost.
We focus on three needs that show up again and again: putting food on the table, keeping a roof overhead, and getting to medical appointments. These are not luxuries. They are the foundation of a healthy life.
